Referral Agency Information

If you are looking to refer a client to Westover Treatment Centre we require a complete assessment package. Please contact our admissions coordinator at slosiak@westovertreatmentcentre.ca if you do not have the Westover specific paperwork.

A complete assessment package includes the following:

  • Catalyst (4 pages)

  • Family Information

  • Legal Charges/Issues

  • Medical Form

    • If the client is on Suboxone we require a letter from the prescribing doctor confirming the client’s dosage, how long they’ve been stable on the dosage and how the client’s 19 day supply will be coming

  • A completed Release of Information

    • The Assessment tools (Only one of the following):

      • interRAI

      • GAIN – Q3RRS

      • ADAT

      • Substance Use Assessment Summary Form

Admission Criteria

  • Client must be physically and mentally capable of fully participating in a structured, intensive treatment program

  • Client should be interested in an abstinence focused treatment centre

  • Client will need to be 7 days free from alcohol and drugs (including marijuana) prior to their provisional date

  • Client cannot be prescribed benzodiazepine medication

  • Client cannot be incarcerated

  • Client cannot be on methadone

  • Client cannot be currently in hospital

  • Clients on Suboxone will need to be on a stable dose and no longer using opioids. The prescribing doctor will need to provide a supporting letter for the client. The Suboxone script will need to be sent to Thamesville Pharmacy (fax # 519-692-9209) prior to the clients arrival.

  • Services will be offered equally to all residents of Ontario 16 and older

  • Assessment must be completed with a referral agency and some restrictions may apply
